Request for Proposals – Graphic Designer

Project Purpose

TEAM UP is seeking a designer to provide graphic design to redesign existing communication materials into polished, versatile design assets for use across external communications. A core requirement of this work is the ability to translate the complexity of TEAM UP’s work, abstract concepts in integrated behavioral health into clear, accessible visuals that clarifies the heart of our work. The designer will collaborate with the project lead to deliver designs that align with the brand guidelines provided and project objectives.

Practice Transformation Coach, TEAM UP Center

The Practice Transformation Coach supports practices in their implementation of the TEAM UP model. As a member of the TEAM UP Center’s implementation team, the Practice Transformation Coach will provide training, consultation, coaching, and technical assistance to practices engaged in the TEAM UP Learning Community. This role will require collaboration with clinical training leads and the TEAM UP Center’s evaluation team to ensure all elements of the implementation strategy are well-aligned. The Practice Transformation Coach will support practice leadership to understand and engage in the relevant subject matter and utilize their data to assess implementation progress and inform ongoing improvement.

Mental Health Clinician (Boston Community Pediatrics)

Boston Community Pediatrics is seeking a Mental Health Clinician to be part of an integrated team of clinicians, pediatric providers, and family partners in order to change the way traditional medical and mental health care is provided to pediatric patients, predominantly with low income.
