Cohort Partnerships

Full model implementation is considered the gold standard for integrated behavioral health care. Practices and collaborating partners within a cohort to implement the TEAM UP model as part of the TEAM UP Learning Community. They receive in-depth clinical training, practice transformation support, and customized, data-driven technical assistance.

Who should consider Model Implementation?

Pediatric primary care or family medicine practices serving children and adolescents.

Why should my practice apply? 

  • Access to care: Ensure prompt access to needed mental and behavioral health services for children and adolescents in your practice   
  • TEAM UP Learning Community: Collaborate with other practices and receive in-depth clinical training   
  • Comprehensive support: Receive customized financial, technical, and operational assistance   
  • Certification as a TEAM UP practice: Gain recognition for your practice and tap into on-going support from the TEAM UP community 

What does Model Implementation look like?

Implementation Phases

Practices implement TEAM UP over two and a half years, organized into three phases:

Planning 6 Months

  • Onboard staff into core team roles
  • Ready EMR systems for data gathering
  • Begin revenue optimization activities

Implementation 18 Months

  • Implement core workflows in the clinical model
  • Participate in Learning Community clinical training and practice transformation support
  • Track progress through the implementation dashboard

Sustainability 6 Months

  • Stabilize and sustain all workflows
  • Complete certification process
  • All TEAM UP practices have ongoing access to the Learning Community
