Director of Practice Transformation

Charlotte Vieira, MPH, is the Director of Practice Transformation for the TEAM UP Center where she leads strategy for the design and provision of technical assistance, consultation, and data use to facilitate implementation and advancement of integrated behavioral health care. As a member of the team since 2019, she has been responsible for establishing and maintaining a structured framework for practice transformation that utilizes data to facilitate change; overseeing the development of resources and platforms to support implementation, improvement, and TEAM UP Model certification; and managing external partnerships across collaborating organizations. Prior to joining TEAM UP, Charlotte supported multiple state-wide initiatives to drive best practice model development, implementation and scale-up of evidence-based practices, and quality improvement as part of The Baker Center's Quality Care Initiative. Charlotte has also worked in direct care, delivering problem solving interventions and case management support to families with young children.
