Model Implementation

Full model implementation is considered the gold standard for integrated behavioral health care. Practices and collaborating partners within a cohort implement the TEAM UP model as part of the TEAM UP Learning Community. They receive in-depth clinical training, practice transformation support, and customized, data-driven technical assistance.
